The Transplant Psychiatry Nurse Practitioner, in collaboration with the Multidisciplinary Team and supervising MD, will provide consultation and follow-up on patients referred for transplant evaluation and ongoing management in all inpatient and outpatient solid organ transplant programs.
Essential Duties:- Rounding on inpatients: providing orders, instructions, education in collaboration with the Supervising Physician(s).
- Management of the health care of the pre and post transplant patient population under the supervision of the attending physician.
- Performance of history and physical examination.
- Order diagnostic testing such as lab, Imaging, EKG and utilize results for formulating treatment plan.
- Review diagnostics studies and take appropriate action.
- Order non-operative therapeutic modalities.
- Prescribe medication as needed for symptom management.
- Complete medication reconciliation as required by CSMC policy.
- Administer medications as privileged and described as part of the plan of care.
- Provide patient/family education; provide patients and family with information on the plan of care.
- Record progress notes.
- Refer to multidisciplinary teams as appropriate.
- Assist the clinical team with triaging phone calls.
- Coordinating the discharging planning of patients and patient discharge education.
- Record discharge summaries.
Qualifications
Qualifications - External
Education:
- MSN Degree required
- Graduate of an accredited Nurse Practitioner program required
- AGACNP preferred
License/Certification:
- Current State of California Registered Nurse and Nurse Practitioner License required
- National certification as an NP required - Psychiatric Mental Health NP
- Current BLS certification from the American Heart Association or American Red Cross required
- DEA license required.
Experience:
- A minimum of 3 years of acute care nursing experience preferred
- Nurse Practitioner with relevant acute care experience preferred
- Psychiatry, Mental Health, Behavioral Health experience preferred
