The Transplant Psychiatry Nurse Practitioner, in collaboration with the Multidisciplinary Team and supervising MD, will provide consultation and follow-up on patients referred for transplant evaluation and ongoing management in all inpatient and outpatient solid organ transplant programs.

Essential Duties:
  • Rounding on inpatients: providing orders, instructions, education in collaboration with the Supervising Physician(s).
  • Management of the health care of the pre and post transplant patient population under the supervision of the attending physician.
  • Performance of history and physical examination.
  • Order diagnostic testing such as lab, Imaging, EKG and utilize results for formulating treatment plan.
  • Review diagnostics studies and take appropriate action.
  • Order non-operative therapeutic modalities.
  • Prescribe medication as needed for symptom management.
  • Complete medication reconciliation as required by CSMC policy.
  • Administer medications as privileged and described as part of the plan of care.
  • Provide patient/family education; provide patients and family with information on the plan of care.
  • Record progress notes.
  • Refer to multidisciplinary teams as appropriate.
  • Assist the clinical team with triaging phone calls.
  • Coordinating the discharging planning of patients and patient discharge education.
  • Record discharge summaries.

Qualifications

Qualifications - External

Education:

  • MSN Degree required
  • Graduate of an accredited Nurse Practitioner program required
  • AGACNP preferred

License/Certification:

  • Current State of California Registered Nurse and Nurse Practitioner License required
  • National certification as an NP required - Psychiatric Mental Health NP
  • Current BLS certification from the American Heart Association or American Red Cross required
  • DEA license required.

Experience:

  • A minimum of 3 years of acute care nursing experience preferred
  • Nurse Practitioner with relevant acute care experience preferred
  • Psychiatry, Mental Health, Behavioral Health experience preferred
