The Transplant Coordinator I are involved with the planning and coordination of routine to moderately complex perioperative and intra-operative transplant process. As a post-transplant coordinator, I will work closely with the medical and surgical team to oversee the post-transplant patient care.
Primary Duties and Responsibilities
- Respond to patient calls and clinical concerns and appropriately document the patient’s medical record.
- Participates with the primary coordinator for the interdisciplinary team, including physicians, nurses, social workers, dietitians, pharmacists, financial analysts, case managers, and support staff to facilitate patient progression through the care continuum.
- Assesses appropriate medical intervention for effective use of resources and quality assurance.
- Collects/documents data to meet clinical, regulatory (OPTN/UNOS/MCS), financial requirements.
- Participates in the development and implementation of practice guidelines, protocols, pathways, performance improvement and research activities.
- Provides nursing care consistent with Nursing Standards of Care/Practice, the Plan for Provision of Nursing Care, The Plan for Provision of Care to Patients, the CSMC Code of Conduct, the Performance Improvement Plan, the Patient Education Plan, and the Nursing Strategic Plan.
- Applies a systemic approach to assess and meet the educational needs of transplant patients.
- Identifies safety concerns and institutes measures to deliver safe patient care; and provides appropriate assessment, intervention, and evaluation of patients with consideration of clinical and developmental breakthroughs in the patient population.
- Identifies the ability of the patients to follow directions/instructions and give consent and intervene appropriately.
- Develops, coordinates, implements, and evaluates educational programs/projects for patients, interdisciplinary team members, and the community. Represents the Medical Center at regional and national meetings/conferences.
- Maintains competency skills, including the ability to deliver care to special patient populations and patients with diverse cultural backgrounds.
- Partners with Organ Procurement Organizations and facilitates the transplant process as organs are available for transplant candidates.
Supports donor management and coordinates transplant recipient care
Qualifications
Education:
- BSN degree required
License/Certifications:
- Current California RN License required
- BLS from the American Heart Association required
Experience:
- A minimum of 3 years of acute care nursing experience required
- Organ transplantation experience preferred
- Experience in data management preferred
- Excellent interpersonal skills and ability to effectively coordinate all aspects of patient care required
