Social Worker - Per Diem -12hr Days - Inpatient/ED Social Services - Marina del Rey Hospital

Job Description

Principal Responsibilities:

  • Perform comprehensive social work assessments and interventions for inpatients and Emergency Department patients.
  • Develop and implement measurable, realistic plans to improve social service delivery.
  • Conduct psychosocial assessments and reassessments for all patient populations, including pediatric, adolescent, geriatric, and general patient populations.
  • Recognize and respond to psychosocial needs of patients and their families, offering crisis intervention and short-term counseling when necessary.
  • Maintain professional relationships with physicians, case managers, nursing staff, county agencies, community resources, and patients/patient representatives.
  • Ensure effective communication at all levels within the hospital system to coordinate social service/discharge planning activities.
  • Document patient care activities according to current standards and policies, optimizing patient safety and reducing medical/health errors.
  • Provide appropriate resources and support to facility staff. Act as a social services liaison, especially for Advance Care Planning, complex discharge planning, and biopsychosocial assessments.
  • Assess behavioral health patients, arrange evaluations, and coordinate necessary follow-up actions.
  • Participate in team rounds and interdisciplinary collaborations, assisting with discharge coordination for substance abuse treatment centers and mental health programs.
  • Apply clinical judgment to recommend solutions for psychosocial barriers to treatment and discharge planning, coordinating discharge for complex cases and under-resourced patients.

Qualifications

Education:

  • Master's Degree in Social Work (Minimum)

Work Experience:

  • 2 years of clinical social work experience (Preferred)
  • Experience in hospital-based, medical settings, and palliative/hospice care (Preferred)
  • 1 year of MSW internship may be considered. Discharge planning experience (Preferred)

Licenses and Certifications:

  • Associate Clinical Social Worker (ASW) (Required)
  • Licensed Clinical Social Worker (LCSW)  (Preferred)
