Job SummaryThe Behavioral Health Coordinator works under the umbrella of care coordination and functions as an essential team member of the Collaborative Care (CoCM) Program. As the cornerstone of this program, the BH coordinator supports both patients and providers alike by facilitating timely patient outreach, triaging patients and referring them to appropriate care, and managing the day-to-day administrative operations of the CoCM program. In this unique position, the BH coordinator will have the opportunity to support our most at-risk patients struggling with mental health concerns get connected to services, and is a crucial part of helping us deliver high quality, holistic care to the Cedars Sinai patient population.
Job duties:- Reviews patient’s chart and triages new referrals to the Collaborative Care Program using inclusion/exclusion criteria
- Makes appropriate referrals to CoCM Program or Ambulatory Care Management (ACM) team based on screening guidelines/criteria
- Facilitates communication between providers/support staff as appropriate
- Conducts patient outreach via phone or EHR which includes (but is not limited to) introducing the patient to the Collaborative Care model, setting expectations, and billing information
- Schedules new patient appointments and reschedules no-shows/cancelled appointments
- Manages BH CoCM program/episodes (day-to-day administrative duties)
- Manages phone calls on the BH Collaborative Care main line
- Responsible for billing for CoCM program services on a timely basis
- Attends regular meetings with the BH Collab Care Team
- Communicates frequently with the Behavioral Health Specialist(s) (BHS) regarding patient care
- Manages the BHS's schedule in the EHR
- Completes daily intervention logs
Qualifications
- High School Diploma/GED required
- 1 year Healthcare experience, a general knowledge of medical terminology and experience with community resources and social supports required
- 1 year Previous utilization management or managed care experience; word processing spreadsheet skills also preferred.

Our compensation philosophy
We offer a competitive compensation package for our employees. The base pay range shown generally applies to the greater Los Angeles area and may vary based on work location. Our range takes into account the wide range of factors that are considered in making compensation decisions, including knowledge/skills: relevant experience and training, education/certifications/licensure; and other business and organizational factors.
