What will you be doing in this role?

The Medical Social Worker is responsible for performing social work assessments and interventions as needed for inpatients and Emergency Department patients. As a member of the multidisciplinary team, they perform comprehensive assessments and assist with developing a plan that integrates medical and social resources. Functions include crisis intervention, patient/family intervention, high-risk screening, and brief counseling, referring to identified resources and engagement of appropriate agencies or community resources when high-risk patients are identified. Establishes and maintains professional relationships with physicians, case managers, nursing staff, county agencies, community resources, patients/patient representatives.

Primary Duties and Responsibilities:

  • Ensures that social service/discharge planning activities are integrated and coordinated within the hospital system by effective communication at all levels.
  • Develops specific, measurable, realistic plans and objectives, which improve social service delivery.
  • Ability to perform a psychosocial assessment and reassessment on all patients. This includes pediatric, adolescent, geriatric and the general patient population. Identifies and addresses psychosocial needs of patients and family.
  • Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health errors. Documentation meets current standards and policies.
  • Demonstrates knowledge of clinical social work and post-acute needs.
  • Interacts professionally with patients/families and involves patients/families in the information of the plan of care.
  • Communicates appropriately and clearly to physicians and all ancillary services. Consults other departments as appropriate to collaborate in patient care and performance improvement activities.
  • Coordinates and directs social services to ensure patients’ needs are met and hospital policy is followed.
  • Provides appropriate resources and support to the facility staff.
  • Performs consultative activities including recommendations for changes in service delivery. Acts as the social services liaison for the department, particularly in the areas of: Advance Care Planning discussions, complex discharge planning, and bio-psychosocial assessments.
  • Assesses behavioral health patients and arranges evaluations, 5150 hold, input psych transfers and/or arrange to output. Follows up as appropriate.
  • Attends team rounds and participate in interdisciplinary collaborations. Assists in addressing recommendations from psychiatry and neuropsychology, including coordinating discharge to substance abuse treatment centers and mental health programs.
  • Applies clinical judgment in making recommendations that will assist in addressing psychosocial barriers to treatment and discharge planning, coordinates discharge for often complex cases and under-resourced patients.

Qualifications

Qualifications:

  • Master’s Degree in Social Work (required).
  • 2 years of clinical social work experience (required).
  • 2 years of experience in hospital based, medical setting, and palliative/hospice care (preferred).
  • 1 year of MSW internship may be considered. Discharge planning experience (preferred).
  • Dedicated to patient and family-centered care that values personal self-determination, skilled at managing multiple priorities at once and engaging in creative, compassionate and ethical problem-solving.
