We are seeking an experienced, knowledgeable Case Manager to work directly in our Emergency Department, collaborating closely with Physicians to determine that most appropriate level of care for a diverse array of patients and medical conditions.

• Provide case management services in collaboration with physicians, CSMC staff, and the Emergency Department
• Primary services include providing consultation regarding patient classification determination (inpatient or observation)
• Recommend alternatives to admission (non-acute resources, alternative levels of care)
• Work with the healthcare team to achieve optimal clinical, financial, and patient satisfaction outcomes
• Identifies and intervenes when plan variances occur
• Identifies practice patterns and quality issues that arise
• Initiates transition of care for patients who will be managed by the inpatient case managers on patient care units

Qualifications:

Education:

• Bachelor of Science, Nursing (BSN) required
• Master's Degree, Nursing (MSN) preferred

License/Certification:

• Current, unrestricted California RN License required
• BLS from the American Heart Association or American Red Cross required
• Certified Case Management RN preferred

Experience:

• A minimum of 5 years of case management experience required
• At least 3 of years of ED nursing experience preferred
• Emergency Department Case Manager experience preferred
• Experience using Interqual or Milliman required
• High level of competency performing medical necessity reviews with accepted criteria required
