Job Description

Job Summary: Key Job Responsibilities
The Executive Director is responsible for overseeing and providing strategic direction and vision to all aspects of Ambulatory Care Coordination and related activities at Cedars-Sinai. The role involves close partnership with physician leaders and payers to determine programmatic priorities and establish high-performing programs and services to serve value-based care across the entire continuum of care. The Executive Director manages staff supporting these functions and serves as an advisor to physicians and providers. This role requires using data to evaluate departmental performance and align with overall value-based metrics of success. The Executive Director implements tactics to improve operational processes, patient outcomes, and financial performance. Ensuring patient safety, engagement, and experience through the highest quality, most impactful programs and services is essential. The position also works collaboratively across the Health System to identify opportunities for program integration and operational synergies. Growth in the position may include direct oversight of departments and staff that serve multiple business lines across different entities within the broader Health System.

Primary Duties and Responsibilities

  • Closely monitor the needs of value-based payer relationships and health system priorities related to capacity management, quality improvement, appropriate use of services ("5 Rights"), patient experience, and provider/staff engagement to ensure that Care Coordination programs and services are meeting the evolving and diverse needs of the organization.
  • Responsible for planning, supervising, coordinating, and successfully executing all activities related to the department's ambulatory operations. Uses data to drive process improvement and achieve outcomes.
  • Set department objectives in alignment with organizational goals and priorities, and communicate program and patient outcomes to upper management.
  • Collaborate closely with hospital-based case management to design systems, workflows, and protocols that ensure high-fidelity hand-offs and effective care transitions in and out of the hospital setting.
  • Coach the management team to apply standard principles in their leadership work and implement the principles of LEAN process improvement.
  • Supervise staff duties and responsibilities, providing continuing education and coaching to ensure the team provides the highest quality patient care with exceptional service.
  • Ensure that schedules accommodate the demands of referring physicians and patients, and make recommendations for adjusting staffing and schedules accordingly.
  • Direct the integration of care coordination services between departments and across the continuum of care to ensure continuous improvement of all clinical practices, services, and operations through the design and implementation of systems, processes, and methods to evaluate and improve patient care.
  • Identify areas for cost containment and work with stakeholders (within and outside the organization) to implement processes that ensure compliance.
  • Collaborate closely with stakeholders to develop plans for physician education and methods for identifying opportunities to address questions and concerns about department programs and services.
  • Advise on and support the implementation of a strategic plan to optimize Care Coordination services throughout the health system and leverage partnerships with affiliates. Oversee and collaborate across multiple business lines and staff associated with affiliate organizations.
  • Work closely with organizational leadership to maintain strong relationships with payers, contractors, and vendors.
  • Implement new programs that service the care transition and avoidable admission processes.
  • Establish standard workflows and use of care management system tools throughout the care coordination department.
  • Maintain dyad partnerships with key physician leaders.
  • Evaluate and incorporate new technology solutions to continuously improve departmental efficiency and effectiveness.

Post Acute/Alternate Site ManagementEnsure appropriate use and oversight of all post-acute and alternate site care, including home health and SNF/custodial care.
Medical Provider NetworkMeet the needs of CS work comp program by ensuring a physician roster and strong case management/UM program for injured workers. Knowledge of ACOEM guidelines.
Population Health ProgramsEnsure efficient and effective workflows for all pop health programs, continuously evaluate the need for current/future pop health programs, recommend, design and implement new programs as needed.


Qualifications

Job Qualifications 

Education

Minimum Requirement - High School Diploma/GED

Minimum Requirement - Bachelor's Degree - in Business Administration ,Nursing or Healthcare 

Preferred Requirement - Master's Degree - in Healthcare related field  

Preferred Requirement - Nursing Degree

Work Experience 

Minimum Requirement - 8 years of Experience - Progressive Healthcare management experience 

Minimum Requirement - 5 years of Experience - Case Management experience 

Licenses and Certifications 

Preferred - RN State License - Current CA License  

Preferred - LVN - Current CA License
