Job Description

Full time hybrid role. Monday - Friday, 8 hours day shift. New hire training requires to be onsite full time approximately 4 - 6 weeks. After training, employee required to be onsite a minimum of 1x a week.

As a full time hybrid Pharmacist in our Refill Center at the Cedars-Sinai Medical Network, you will be providing clinical pharmacy services in all pharmacist-managed programs at Cedars-Sinai Medical Network and ensuring patient safety and the highest quality of care is delivered.

  • Providing therapeutic recommendations and answering medication and patient-related questions from other healthcare providers
  • Identifies, resolves, and prevents potential and actual medication-related problems including untreated indications, improper drug selection, sub-therapeutic dosage, overdosage, failure to receive medication, adverse drug reactions, drug interactions, and medications without indication
  • Provide patients’ and other healthcare providers with education on medication use, side effects, and drug interactions when appropriate
  • Communicate with physicians and pharmacists to recommend changes in medication therapy to optimize patient outcomes (i.e. labs not at goal, discontinuation of medication)
  • Support the process of completing medication prior authorizations as needed
  • Thorough documentation of patient progress and outcomes for future reporting
  • Assures ongoing performance improvement of the pharmaceutical care services provided
  • Providing physician education through presentations and "Pharmacy Hot Topics" Cedars-Sinai Medical Group internal monthly newsletter
  • Training pharmacy residents and students rotating through CSMG
  • Actively participate in meetings within the department
  • Recommends changes in drug therapy based on information obtained through patient interviews, objective monitoring parameters, review of drug profiles and the patient medical record
  • Evaluates herb/supplement and drug/disease interactions
  • Recommends evidence-based integrative medicine options as appropriate for patient-specific needs
  • Approve protocol medication refill requests under physician signed protocols on behalf of CSMCF physicians
  • Identify patients for Drug Therapy Management Programs
  • Promote cost-effective drug therapy by converting brand to generic medications when appropriate, per physician signed protocol
  • Track interventions to show the value of the Refill Center
  • Ensure medication refills are completed in a timely manner
  • Evaluation of drug therapies, development of treatment plans, initiation, titration/tapering and discontinuation of medications under protocol, ordering necessary lab tests under protocol, patient education and follow-up
  • Thorough documentation of patient progress and outcomes for future reporting
  • Participate in medication related quality initiatives as needed
  • Collaborates to problem solve and make decisions to achieve desired outcomes

Qualifications

Educational Requirements:

  • A Doctor of Pharmacy (Pharm. D.) required

License/ Certifications Requirements:

  • Current California Pharmacist license required, National Provider Identifier (NPI) preferred

Experience:

  • Retail Pharmacy experience highly preferred.
  • Comprehensive knowledge of medical terminology as related to drug usage;
  • Comprehensive knowledge of common disease states, pathologies and ability to apply current pharmacological and biopharmaceutical principles;
  • Ability to comprehend patient medical management plans;
  • Possess excellent verbal and written communication skills;
  • Ability to give advice on and monitor the use of drugs;
  • Knowledge of laws, regulations and standards relative to Institutional Pharmacy Practice and prescription dispensing;
  • Ability to prioritize workflow;
  • Ability to recommend, develop, and expand programs;
  • Ability to learn and adapt to the use of automated information systems in daily practice;
  • Demonstrated competency in pharmaceutical calculations and pharmacokinetics;
  • Basic computer proficiency including Windows, Word, Excel, Power Point and Outlook.
