Our Nurse Practitioner's practice incorporates both independent decision-making abilities and collaboration with the board certified/eligible physician to provide expert care to the patients. This role enhances competency skills through continuing education programs. This role has the ability to provide care to special patient population and those with diverse cultural background. Conduct physical examinations, order and interpret diagnostic imaging and laboratory studies, prescribe medications as appropriate, and coordinate referrals to physical therapy, behavioral health/mental health services, and other supportive care resources to ensure comprehensive, patient-centered care.
- Conduct comprehensive assessments of clinically complex patients, evaluating medical, functional, psychosocial, cognitive, nutritional, cultural, and educational needs to develop individualized care plans. Collaborate with patients, families, and the interdisciplinary care team to coordinate and communicate treatment plans.
- Perform diagnostic evaluations and therapeutic interventions in accordance with established clinical protocols. Accurately document procedures, treatment plans, patient progress, and follow-up care in the electronic medical record.

- Provide individualized patient and family education on disease prevention, treatment plans, medication management, lifestyle modifications, smoking cessation, vaccinations, survivorship care, and other condition-specific topics. Assess readiness to learn, tailor education to each patient's needs and health literacy, and reinforce teaching to promote informed decision-making and self-management.
